Lumbar puncture timing with enoxaparin
Lumbar puncture is supported only after an adequate interval from the last enoxaparin dose to reduce spinal hematoma risk [1]. For enoxaparin given for VTE prophylaxis, the interval is at least 12 hours [1]. For enoxaparin given at therapeutic dosing, the interval is at least 24 hours [1].
Enoxaparin hold interval before lumbar puncture
- Prophylactic LMWH dosing: hold enoxaparin and perform lumbar puncture ≥12 hours after the last dose [1].
- Therapeutic LMWH dosing: hold enoxaparin and perform lumbar puncture ≥24 hours after the last dose [1].
Prophylactic versus therapeutic dosing definitions used for timing
- Prophylactic dosing is managed using the LMWH “prophylactic dose” timing schedule with a 12-hour interval before neuraxial procedures or lumbar puncture [1].
- Therapeutic dosing is managed using the LMWH “treatment dose” timing schedule with a 24-hour interval before neuraxial procedures or lumbar puncture [1].
Post-lumbar puncture restart considerations
- Guidance for restarting enoxaparin after neuraxial procedures is addressed separately from the pre-procedure hold interval and should follow local perioperative protocols and the relevant timing schedule for catheterless lumbar puncture [1].
