Tadalafil Adverse Effects: Dizziness and Diarrhea

Dizziness and diarrhea are known adverse reactions of tadalafil. [1] In an elderly patient, these symptoms should be treated as potential blood pressure–lowering and/or dehydration adverse effects and tadalafil should be withheld pending clinical assessment. [1]

Immediate Symptom Assessment

Vital signs should be assessed, with particular attention to orthostatic blood pressure and pulse, given the blood pressure–lowering effects associated with tadalafil. [1] Hydration status should be assessed because diarrhea can contribute to volume depletion. [1] Review should be performed for concurrent agents that can increase hypotension risk, including nitrates, alpha-blockers, and antihypertensives. [1]

Medication Management

Tadalafil should be discontinued temporarily while dizziness and diarrhea are active, pending evaluation of severity and contributing drug interactions. [1] Nitrates should not be used concurrently with tadalafil because the combination is contraindicated due to potentiation of hypotensive effects. [1] Caution is required when tadalafil is used with alpha-blockers because additive blood pressure lowering can occur. [1]

Supportive Care for Diarrhea and Volume Depletion

Oral rehydration and general supportive care should be provided for diarrhea to reduce dehydration-related dizziness. [1] Persistent or severe diarrhea with ongoing dizziness should prompt escalation of care for evaluation of hypotension and complications from volume depletion. [1]

Safety Escalation Criteria

Emergency medical evaluation is indicated for syncope, severe or persistent dizziness with concern for significant hypotension, or other severe systemic symptoms during tadalafil exposure. [1] Emergency evaluation is indicated for serious cardiovascular symptoms temporally associated with tadalafil use. [1]

After Symptom Resolution and Restart Planning

After dizziness and diarrhea resolve, tadalafil restart should be considered only after addressing contributing factors, including medication interactions, hydration status, and organ function. [1] If tadalafil is restarted, dose selection and interval should follow prescribing guidance and account for interaction risk and patient-specific exposure factors. [1]
