Ondansetron (Zofran) Ingestion in a 2-Year-Old

A 2-year-old who ingests an ondansetron tablet should have immediate poison-center guidance and monitoring for symptoms. [1] Ondansetron overdose management is primarily supportive care. [2]

Immediate Actions

Call Poison Control at 1-800-222-1222 for real-time, ingestion-specific instructions based on dose, time, and symptoms. [1] Do not delay calling Poison Control for symptom onset. [3] Keep the medication container available and be ready to provide the product name, strength, approximate number of tablets, and time since ingestion. [4]

Emergency Escalation Criteria

Call 911 immediately if the child has seizures, is very drowsy or difficult to awaken, collapses, has trouble breathing, or cannot stay awake. [3]

Decontamination and What to Avoid

Do not induce vomiting. [4] Syrup of ipecac is not recommended. [4]

Clinical Assessment and Monitoring

Ondansetron overdose can cause adverse effects including hypotension and neurologic toxicity reports including serotonin syndrome after inadvertent oral overdoses exceeding an estimated 5 mg/kg in young children. [2] Because ondansetron is associated with QT prolongation risk in general clinical use, clinicians should consider electrocardiographic assessment when overdose is significant or symptoms occur (eg, syncope, palpitations, abnormal mental status). [5] Supportive care is recommended for symptomatic children. [2]

Treatment Approach

If the child remains asymptomatic, Poison Control guidance typically determines whether observation at home versus emergency evaluation is indicated based on estimated ingested dose and time since ingestion. [1] If the child is symptomatic, emergency evaluation is indicated for monitoring and supportive therapy. [2]

Key Information to Provide Poison Control

The number of tablets ingested should be determined if possible. [1] The tablet strength (mg per tablet) should be identified from the package. [4] The exact time of ingestion should be provided. [4]

Medication Safety Prevention

Medication should be stored out of reach of children in locked or otherwise secure containers. [1]

Disposition and Follow-Up

Poison Control instructions should be followed for observation, need for emergency evaluation, and return precautions. [1] If any concerning symptoms develop during monitoring, immediate emergency care should be sought. [3]
