Adderall Immediate-Release to Extended-Release Conversion
Patients taking divided doses of immediate-release ADDERALL (for example, twice daily) may be switched to ADDERALL XR at the same total daily dose taken once daily. [1]
Dose-Equivalence Rule (Total Daily Dose)
- If ADDERALL IR 10 mg is taken once daily (total daily dose 10 mg), ADDERALL XR should be 10 mg once daily in the morning. [1]
- If ADDERALL IR 10 mg is taken twice daily (total daily dose 20 mg), ADDERALL XR should be 20 mg once daily in the morning. [1]
Administration Timing During Conversion
ADDERALL XR should be given upon awakening, and afternoon doses should be avoided due to potential insomnia. [1]
Titration After Switching
Dose adjustment should occur at weekly intervals based on efficacy and tolerability. [1]
Children and Adolescents Starting/ Switching Dose Context
- For children ages 6–12 years switching from another medication, the recommended starting dose is 10 mg once daily in the morning. [1]
- For adolescents ages 13–17 years switching from another medication, the recommended starting dose is 10 mg/day, with possible increase to 20 mg/day after one week if ADHD symptoms are not adequately controlled. [1]
Practical Switching Constraints
ADDERALL XR should use the lowest effective dosage. [1]
