Switching From Vilazodone to Sertraline (Cross-Taper)
Cross-tapering is recommended for switching between antidepressants when both agents can be used briefly during transition. [1] Vilazodone discontinuation should be down-tapered rather than abruptly stopped. [2] Sertraline should be initiated at 50 mg daily with titration by 50 mg at least weekly. [3]
Medication Selection Algorithm
The switch is between a serotonergic antidepressant (vilazodone) and an SSRI (sertraline). [2] A cross-taper strategy should be used with overlapping dosing of both agents during vilazodone down-taper. [1]
Cross-Titration Schedule for Vilazodone 40 mg to Sertraline 100 mg
Day 1
- Vilazodone should be reduced from 40 mg once daily to 20 mg once daily with food. [2]
- Sertraline should be initiated at 50 mg once daily. [3]
Days 1–4
- Vilazodone should remain at 20 mg once daily. [2]
- Sertraline should remain at 50 mg once daily. [3]
Days 5–7
- Vilazodone should be reduced to 10 mg once daily. [2]
- Sertraline should remain at 50 mg once daily. [3]
Day 8
- Vilazodone should be discontinued. [2]
- Sertraline should be increased to 100 mg once daily. [3]
Treatment Initiation Thresholds
Sertraline titration should follow a weekly schedule of at least 50 mg increments. [3] Vilazodone discontinuation should follow a gradual down-taper over 7 days from 40 mg once daily to discontinuation. [2]
Common Pitfalls to Avoid
Abrupt discontinuation of vilazodone should be avoided due to risk of discontinuation symptoms. [2] Co-administration should be avoided only when a contraindicated medication switch involves an MAOI strategy; MAOI washout is not applicable to this SSRI-to-SSRI cross-taper. [1]
