Cefdinir dosing for bacteremia in adults and children with renal impairment

Cefdinir dosing recommendations are provided for infections treated with cefdinir. Cefdinir dosing is adjusted for markedly compromised renal function based on creatinine clearance. [1]

Adults with normal renal function

For adults and adolescents age 13 years and older, the total daily cefdinir dose for infections is 600 mg. [1]

  • 300 mg every 12 hours (q12h) for a total daily dose of 600 mg. [1]
  • 600 mg once daily (q24h) is an alternative regimen for some infections. [1]

Adults with impaired renal function

Creatinine clearance <30 mL/min is the renal threshold for cefdinir dose reduction. [1]

  • Creatinine clearance <30 mL/min: cefdinir 300 mg once daily. [1]

Patients on hemodialysis

  • Initial regimen in chronic hemodialysis: 300 mg every other day. [1]
  • After each hemodialysis session: 300 mg should be given. [1]

Pediatric patients (6 months through 12 years)

Total daily cefdinir dose for pediatric patients with normal renal function is 14 mg/kg/day, up to a maximum daily dose of 600 mg. [2]

  • 7 mg/kg q12h (total 14 mg/kg/day). [2]
  • 14 mg/kg q24h (total 14 mg/kg/day). [2]

Pediatric patients with impaired renal function

  • Pediatric patients with creatinine clearance <30 mL/min/1.73 m²: cefdinir 7 mg/kg (up to 300 mg) once daily. [2]
