Rabies Post-Exposure Prophylaxis Administration Site Compatibility
Rabies vaccine and human rabies immune globulin (HRIG) should not be administered in the same syringe or in the same anatomical site. [1] HRIG should be infiltrated into and around wounds when possible. [1] Any remaining HRIG volume should be given intramuscularly at an anatomical site distant from the rabies vaccine administration site. [1]
Primary Administration Rule
HRIG must be given at a site separate from the first rabies vaccine dose site. [1] HRIG must never be administered in the same syringe as rabies vaccine. [1]
Practical Interpretation for “Same Arm”
Administering HRIG and rabies vaccine in the same arm is acceptable only when they are placed in different anatomical locations within the arm. [1] If the vaccine has been given in the deltoid area, HRIG should be administered into and around the wound(s) or into an anatomically distant site, rather than into the vaccine injection site. [1] If HRIG and vaccine are already given in the same anatomical site, vaccine re-administration in a distant location may be required. [2]
Recommended Injection Sites Within the Standard Regimen
Rabies vaccine is administered intramuscularly in the deltoid area for adults. [1] For children, anterolateral thigh administration of vaccine is acceptable. [1] HRIG is infiltrated around bite wounds when possible, with any remainder given intramuscularly at a site distant from vaccine administration. [1]
Common Errors to Avoid
Using the same syringe for HRIG and vaccine is incorrect. [1] Giving HRIG in the same anatomical site as the first rabies vaccine dose is incorrect because product neutralization can occur. [2]
Subsequent Vaccine Doses After Day 0
Subsequent rabies vaccine doses in the series can be administered in the same anatomic location where the HRIG dose was administered. [1]
References and Clinical Implementation
CDC rabies post-exposure prophylaxis guidance includes the requirement that HRIG not be given in the same syringe or anatomical site as the first vaccine dose. [1] CDC biologics guidance reiterates that HRIG neutralizes vaccine when given at the same site. [2]
